Below is a form that asks questions about what kind of a web site you need.

I bill $20 an hour for Web and graphic design services. The cost of your Web site will be greatly reduced if you have a clear vision of what you want, you supply the text for your Web site in Microsoft Word files, and you provide ready to use graphics files. If the above 3 factors are true, I should be able to build you a simple Web site with background textures and a logo for about $240.

If you don’t have an artistic vision to guide my design, you need me to write the text for your site, you need me to scan and optimize the quality of images, or you want me to create animations, the cost of your site will increase considerably. If you want programming services like JavaScript, MySQL databases, and PHP, my time investment (and hence your cost) will increase too.

The questions on this form are color coded. Questions that you must answer in order for this form to submit are in black. Optional questions are in blue. If you answer yes to wanting a database additional questions will become visible. If you answer yes to wanting a commercial web site, the question regarding your business category will turn from blue to red. These changes in page appearance are accomplished with JavaScript.
